Roux-en-Y Gastric Bypass
Gold standard bariatric procedure — small gastric pouch with intestinal bypass for maximum weight loss and diabetes remission.
What is Roux-en-Y Gastric Bypass?
Roux-en-Y Gastric Bypass creates a small 30 ml gastric pouch connected to a rerouted segment of small intestine in a Y-configuration. This simultaneously restricts intake, reduces hunger hormones, and limits calorie absorption, producing the most consistent long-term weight loss and the highest Type 2 Diabetes remission of any bariatric procedure. Dr. Tagore Mohan Grandhi performs Roux-en-Y Gastric Bypass laparoscopically, drawing on fellowship training at Legacy Health System and the University of Kentucky, USA.
How the Procedure Works
Gastric Pouch Formation
Small 30 ml stomach pouch created from the upper stomach using a laparoscopic stapler, completely divided from the remnant stomach.
Roux Limb Creation
Small intestine divided 30–50 cm from the ligament of Treitz. Alimentary (Roux) limb measured to 100–150 cm.
Gastrojejunostomy
Alimentary limb connected to the gastric pouch by a stapled or hand-sewn gastrojejunostomy, bypassing the remnant stomach and duodenum.
Biliopancreatic Anastomosis
Biliopancreatic limb reconnected to the Roux limb downstream (jejunojejunostomy), completing the Y-configuration.
Leak Testing & Recovery
All anastomoses tested intraoperatively. Liquid diet from day one. Lifelong vitamin supplementation. 3-monthly metabolic review.
