What is Fundoplication (Open)?
Fundoplication is the surgical cure for Gastroesophageal Reflux Disease (GERD). The upper stomach (fundus) is wrapped around the lower oesophageal sphincter (LES), restoring the anti-reflux barrier and preventing acid from flowing back into the oesophagus. The open approach is used for complex or revision cases. Dr. Tagore Mohan Grandhi performs fundoplication. Pre-operative oesophageal manometry guides the choice between a 360° Nissen wrap or a 270° Toupet partial wrap. Over 90% of patients are free from reflux and medications long-term.
How the Procedure Works
Pre-operative Workup
Upper GI endoscopy grades oesophagitis and hiatal hernia. Oesophageal manometry and 24-hour pH monitoring performed. Manometry guides wrap selection.
Hiatal Dissection
Upper midline incision. Gastro-oesophageal junction mobilised. Hiatal hernia reduced and hiatus closed with non-absorbable sutures.
Fundic Wrap
Gastric fundus wrapped posteriorly around the LES — Nissen 360° or Toupet 270° based on manometry — and secured with non-absorbable sutures.
Closure
Wound closed in layers. The patient commenced on liquid diet day one, progressing to soft diet over 4–6 weeks.
Follow-up
Endoscopy at 3 months. pH monitoring if symptoms persist. Annual long-term review to confirm durable reflux control.
