What is Appendectomy?
Open appendectomy is the traditional surgical treatment for acute appendicitis, performed through a right iliac fossa (McBurney's) incision. It remains the preferred approach for perforated appendicitis with dense adhesions, when laparoscopic access is not feasible, or when laparoscopic conversion is required during surgery. The open approach provides direct access for dense inflammatory adhesions, thorough pericaecal abscess drainage, and conversion from laparoscopic appendectomy when anatomy is complex.
How the Procedure Works
Emergency Assessment
Alvarado score, blood tests, and CT scan confirm appendicitis and assess for perforation or phlegmon.
McBurney's Incision
Transverse or oblique incision made at McBurney's point in the right iliac fossa. Muscles split in layers under direct vision.
Appendix Delivery
Caecum delivered into the wound. Mesoappendix divided with haemostasis.
Appendix Removal
Appendix base ligated and divided. Stump inverted or ligated. Wound irrigated.
Closure
For perforated cases, thorough irrigation and drain placement. Wound closed in layers. Recovery in 2–4 days.
