What is Primary Bowel Resection?

Primary bowel resection is the surgical removal of a diseased bowel segment followed by immediate reconnection (anastomosis) of the two healthy ends, restoring digestive continuity without a stoma. It is the preferred approach when conditions allow safe anastomosis. Dr. Tagore Mohan Grandhi performs primary bowel resection for Crohn's disease, diverticulitis, colon cancer, bowel tuberculosis, and other conditions. The laparoscopic approach is used wherever feasible.

Suitable for patients requiring bowel resection whose bowel is healthy on both sides, who are haemodynamically stable, in whom primary anastomosis can be performed at acceptable risk.

How the Procedure Works

1

Pre-operative Assessment

CT scan defines disease extent and confirms anastomosis suitability. Bowel preparation and antibiotic prophylaxis administered.

2

Laparoscopic Mobilisation

Affected bowel mobilised laparoscopically. Mesenteric blood supply identified, secured, and divided with haemostasis.

3

Bowel Resection

Diseased segment excised with clear margins. Specimen sent for histopathological analysis.

4

Anastomosis

Healthy bowel ends reconnected by a stapled or hand-sewn join. Anastomotic integrity tested intraoperatively by air or methylene blue.

5

Recovery

Oral fluids on day two, solid diet days three to four. Discharged in 4–7 days. Post-operative histology guides further management.

Who Needs This Treatment?

  • Single operation restoring normal bowel continuity — no stoma
  • Laparoscopic approach — smaller incisions, less pain, faster recovery
  • Avoids a second reversal operation compared to staged surgery
  • Histopathology confirms diagnosis and surgical margins
"

Primary resection with anastomosis is our goal whenever achievable safely. Patients recover faster, avoid a stoma, and return to normal life sooner. Careful patient selection is the key.
