What is Laparoscopic Appendectomy?

Laparoscopic appendectomy is the gold standard treatment for acute appendicitis. Through 3 small keyhole incisions under general anaesthesia, the inflamed appendix is identified, ligated, and removed. It offers significantly less pain, a shorter hospital stay, lower wound infection rates, and faster return to normal activities compared to open appendectomy. Even for perforated appendicitis, the laparoscopic approach allows thorough four-quadrant peritoneal lavage under magnification.

How the Procedure Works

Emergency Assessment

Alvarado score, blood tests, and CT scan confirm appendicitis and assess for perforation or peritonitis.

Laparoscopic Access

Three incisions: 12 mm umbilical, 5 mm suprapubic, 5 mm left iliac fossa. CO₂ insufflation creates working space.

Appendix Identification

Caecum and appendix visualised. Position confirmed. Severity of inflammation and peritoneal contamination assessed.

Appendix Removal

Mesoappendix divided with harmonic scalpel. Appendix base ligated with Endoloop or stapler. Appendix placed in retrieval bag.

Lavage and Extraction

For perforated cases, thorough four-quadrant washout performed. Appendix extracted in bag without wound contact.
