What is Injection Sclerotherapy?
Injection sclerotherapy is a minimally invasive outpatient procedure for Grade I and II internal haemorrhoids in which a sclerosant solution — typically 5% phenol in almond oil — is injected into the submucosa at the base of each haemorrhoidal column. The sclerosant causes an inflammatory reaction leading to fibrosis, shrinkage, and fixation of the mucosa. How the Procedure Works
Pre-procedure Assessment
Proctoscopy confirms Grade I or II internal haemorrhoids and excludes other causes of rectal bleeding.
Patient Positioning
Left lateral or lithotomy position. Proctoscope inserted and haemorrhoidal columns visualised at the 3, 7, and 11 o'clock positions. No anaesthesia required.
Sclerosant Injection
Fine needle inserted through the proctoscope into the submucosa at the apex of each haemorrhoidal column above the dentate line. 2–3 mL of 5% phenol in almond oil injected at each site.
Post-injection Assessment
Mucosal bleb checked to confirm correct submucosal placement. Injection into the muscle layer avoided.
Recovery and Follow-up
Patient observed for 15 minutes and discharged the same day. Mild rectal discomfort for 24–48 hours is common. Response assessed at 4–6 weeks.
