What is Splenectomy?

Splenectomy is the surgical removal of the spleen. Indications include immune thrombocytopenia (ITP) refractory to medical therapy, hereditary haemolytic anaemias (spherocytosis), splenic trauma, lymphoma involving the spleen, and splenic artery aneurysm. Laparoscopic splenectomy is preferred for most elective cases. Dr. Tagore Mohan Grandhi performs laparoscopic splenectomy through 4–5 keyhole incisions. Pre-operative vaccination against encapsulated bacteria (pneumococcal, meningococcal, Hib) — administered at least 2 weeks before surgery — is mandatory. Lifelong oral penicillin prophylaxis is commenced on discharge.

Suitable for patients with immune thrombocytopenia (ITP), hereditary haemolytic anaemia, splenic trauma, lymphoma or haematological malignancy involving the spleen, or splenic artery aneurysm.

How the Procedure Works

Pre-operative Vaccination

Mandatory pneumococcal, meningococcal, and Hib vaccinations given at least 2 weeks before surgery.

Laparoscopic Access

4–5 keyhole incisions. Patient positioned right lateral decubitus to bring spleen into optimal operative field under gravity.

Splenic Hilar Dissection

Gastrosplenic and splenorenal ligaments divided. Splenic artery and vein ligated and divided at the hilum.

Splenic Mobilisation

Spleen fully freed from all ligamentous attachments. Placed in retrieval bag.

Extraction and Prophylaxis

Bag extracted through small extension of largest port incision. Oral penicillin commenced on discharge.
