What is Seton Placement?
A seton is a thread or suture passed through the fistula tract and tied loosely as a loop. It is used as a staged approach for complex high fistulas, maintaining drainage, controlling infection, and allowing post-inflammatory fibrosis before definitive sphincter-preserving surgery. Dr. Tagore Mohan Grandhi uses seton placement as the first stage for complex and Crohn's-related fistulas. The seton is kept in place for 6–12 weeks while inflammation resolves and biologic therapy is established before definitive closure.
How the Procedure Works
Examination Under Anaesthesia (EUA)
Fistula anatomy confirmed under anaesthesia. All tracts, secondary extensions, and internal opening identified. Entire tract probed.
Seton Threading
Non-absorbable thread (silastic sloop or 0 PDS) passed through the tract from external to internal opening.
Loose Tie
Seton tied loosely as a comfortable loop — maintains drainage without cutting through muscle.
Biologic Therapy
For Crohn's fistulas, biologic therapy commenced and continued with seton in situ to control inflammation.
Definitive Surgery Planning
Definitive sphincter-preserving procedure planned at 6–12 weeks once tract is fibrosed, quiescent, and ready.
