Laparoscopic liver cyst fenestration (deroofing) is the definitive treatment for large symptomatic simple hepatic cysts. The entire exposed cyst roof is excised through 3–4 small keyhole incisions, creating a wide opening into the peritoneal cavity. Removing the secreting roof prevents fluid reaccumulation in over 90% of cases. Dr. Tagore Mohan Grandhi performs laparoscopic cyst deroofing. Tumour markers (CA 19-9, CEA) are checked pre-operatively to exclude a biliary cystadenoma, which requires complete excision rather than deroofing.
How the Procedure Works
Tumour Marker Exclusion
CA 19-9 and CEA measured. EUS cyst fluid analysis excludes cystadenoma or neoplasm requiring complete excision rather than deroofing.
Laparoscopic Access
3–4 keyhole incisions. Liver and cyst surface assessed. Cyst location confirmed suitable for laparoscopic deroofing.
Cyst Aspiration
Cyst partially aspirated to decompress and improve visualisation of the cyst wall margins.
Cyst Deroofing
Entire exposed cyst roof excised with scissors and diathermy. Wide opening created into the peritoneal cavity.
Wall Treatment
Argon beam or diathermy applied to remaining cyst wall to destroy residual epithelium. Discharged day 1–2.
