Laparoscopic Liver Cyst Fenestration (Deroofing)

Keyhole removal of the liver cyst roof — definitive treatment for large symptomatic simple hepatic cysts.

What is Laparoscopic Liver Cyst Fenestration (Deroofing)?

Laparoscopic liver cyst fenestration (deroofing) is the definitive treatment for large symptomatic simple hepatic cysts. The entire exposed cyst roof is excised through 3–4 small keyhole incisions, creating a wide opening into the peritoneal cavity. Removing the secreting roof prevents fluid reaccumulation in over 90% of cases. Dr. Tagore Mohan Grandhi performs laparoscopic cyst deroofing. Tumour markers (CA 19-9, CEA) are checked pre-operatively to exclude a biliary cystadenoma, which requires complete excision rather than deroofing.

Suitable for patients with symptomatic simple hepatic cysts causing abdominal discomfort, early satiety, or compressive symptoms, where cyst fluid analysis confirms benign simple cyst.

How the Procedure Works

1

Tumour Marker Exclusion

CA 19-9 and CEA measured. EUS cyst fluid analysis excludes cystadenoma or neoplasm requiring complete excision rather than deroofing.

2

Laparoscopic Access

3–4 keyhole incisions. Liver and cyst surface assessed. Cyst location confirmed suitable for laparoscopic deroofing.

3

Cyst Aspiration

Cyst partially aspirated to decompress and improve visualisation of the cyst wall margins.

4

Cyst Deroofing

Entire exposed cyst roof excised with scissors and diathermy. Wide opening created into the peritoneal cavity.

5

Wall Treatment

Argon beam or diathermy applied to remaining cyst wall to destroy residual epithelium. Discharged day 1–2.

Who Needs This Treatment?

  • Removes the secreting epithelium permanently — 90%+ long-term success
  • 3–4 keyhole incisions — 1–2 day stay, return to activity within 1 week
  • Histopathology of cyst wall confirms benign diagnosis
  • Definitive treatment for large symptomatic simple hepatic cysts
"

Laparoscopic deroofing removes the source of cyst fluid production permanently. Over 90% of patients are free from cyst recurrence and symptoms long-term, through just 3–4 small incisions and a 1–2 day hospital stay.
