Incision and Drainage of Pilonidal Abscess
Emergency drainage of an acute pilonidal abscess — essential first-line treatment providing rapid relief.
What is Incision and Drainage of Pilonidal Abscess?
An acute pilonidal abscess is a sudden onset of severe throbbing pain, redness, and swelling in the natal cleft above the anus. It must be drained urgently under local or general anaesthesia — antibiotics alone are insufficient. Definitive pilonidal sinus excision is planned 6–8 weeks after the abscess has fully healed. Dr. Tagore Mohan Grandhi performs urgent pilonidal abscess drainage providing dramatic pain relief within 24 hours. After drainage and healing, off-midline flap surgery (Karydakis or Limberg) is offered to permanently cure the underlying sinus.
How the Procedure Works
Assessment
Clinical examination confirms fluctuant abscess. Extent of cellulitis noted. Anaesthesia planned based on abscess size and depth.
Anaesthesia
Local anaesthetic for small superficial abscesses. General anaesthesia for large or extensive collections.
Incision and Drainage
Adequate incision over abscess. Cavity opened widely. Pus drained. Sinus pits and granulation tissue curetted to remove hair and debris.
Wound Packing
Cavity packed with antiseptic ribbon gauze. Daily repacking until healing commences.
Definitive Surgery Planning
Definitive excision with off-midline flap reconstruction (Karydakis or Limberg) planned at 6–8 weeks after complete healing.
Outcomes
Who Needs This Treatment?
- →Dramatic pain relief within 24 hours of drainage
- →Antibiotics alone cannot drain an abscess — surgery is the only definitive treatment
- →Same-day procedure under local anaesthesia — no hospital admission
- →Allows planning of definitive sinus excision in a controlled, non-infected setting
Drainage is the most important first step for an acute pilonidal abscess — immediate, effective, and dramatically relieving. After healing, we plan the definitive flap operation to prevent recurrence.
