What is Incision and Drainage of Ischiorectal Abscess?

A perianal or ischiorectal abscess is a painful, rapidly progressive collection of pus near or around the anus caused by infection of the anal glands. It is a surgical emergency — antibiotics alone cannot drain an established abscess, and delay risks spreading infection and necrotising fasciitis. Dr. Tagore Mohan Grandhi performs urgent incision and drainage, providing dramatic pain relief within 24 hours. Following drainage, close surveillance at 3–6 months is arranged to identify and treat any developing fistula tract.

Suitable for patients with perianal or ischiorectal abscess with fluctuant swelling and fever — requiring immediate incision and drainage under local or general anaesthesia.

How the Procedure Works

1

Urgent Assessment

Clinical examination confirms abscess. MRI arranged for deep or horseshoe abscesses not fully defined clinically.

2

Anaesthesia

Local anaesthetic for small superficial abscesses. General anaesthesia for large deep abscesses.

3

Incision and Drainage

Adequate incision directly over the point of maximum fluctuance. Abscess cavity opened widely. Loculations broken to ensure complete drainage.

4

Wound Packing

Cavity packed with antiseptic wick to prevent premature closure. Sitz baths for 1–2 weeks.

5

Fistula Surveillance

Close follow-up at 3–6 months. 30–40% develop a fistula requiring VAAFT, FiLaC, LIFT, or fistulotomy depending on complexity.

Perianal abscess is a surgical emergency — every hour of delay increases pain, infection spread, and fistula risk. Prompt, complete drainage is the most important intervention we can provide, and relief is dramatic and immediate.
