What is Fissurectomy?
Fissurectomy is the surgical excision of the chronic anal fissure, including the fibrous base, sentinel skin tag, and hypertrophied anal papilla. It is typically combined with lateral internal sphincterotomy to address both the diseased tissue and the elevated sphincter tone preventing healing. How the Procedure Works
Anaesthesia
Short general or spinal anaesthesia with local anaesthetic supplementation for post-operative analgesia.
Fissure Identification
Chronic fissure, sentinel skin tag, and hypertrophied papilla identified. Extent of excision planned under anaesthesia.
Fissure Excision
Chronic fissure, sentinel tag, and fibrous base excised using diathermy. Wounds left open to heal by secondary intention.
Sphincterotomy Addition
Lateral internal sphincterotomy added to normalise sphincter pressure and improve blood flow to the excision site.
Wound Healing
Healing by secondary intention over 4–6 weeks. Sitz baths twice daily and a high-fibre diet maintained throughout.
